How does it work?
If you're an eligible Home Care Package participant with funding, you can sign up with a Home Care Package provider who will assess your specific needs and allocate you relevant support services. Home Care Package case managers will seek service providers such as Physiotherapists & Exercise Physiologists to provide you with in-home treatment
